Ifso the Health Improvement Practitioner Service are looking for a new Deputy Service Manager and we would like to hear from YOU!

The Health Practitioner Service has been an established part of Bolton's primary care workforce for over a decade. During this time, we've helped tens of thousands of patients to prevent illnesses like type 2 diabetes, cardiovascular disease, and COPD through our 12 month, evidence-based lifestyle intervention.

As our Deputy Service Manager, you will support a team of Health Improvement Team leaders and Social Prescribing Link Worker all of whom work across 49 GP surgeries in Bolton.

It is ademanding, but rewarding role in what can be a rapidly evolving professional landscape. You will be well supported by the Service Manager and those around you but the ability to cope with and lead others through complex change is crucial for this role.

This is an exciting opportunity to join a thriving team and we look forwarding to hearing from you!, Develop and maintain effective working relationships with the Band 7 Service Manager, HIP Team Leaders, SPLWs and other HIPs Develop and maintain effective working relationships with primary care and Integrated Care Services Division colleagues.

Develop and maintain an extensive knowledge of the HIP role.

Act as the main administrator for the Data Collection and Reporting System (DCRS)

Work collaboratively with the Team Leaders to ensure robust reporting systems, including data input.

Plan, implement, monitor and evaluate robust evaluation systems for the service as defined by the Service Manager.

Support other team members with research, audits and evaluations as required Support the Public Health, Programme Manager for Health Improvement and Primary Care Development , with the development of new wellbeing initiatives for primary care.

Assess and evaluate complex data. Liaise when appropriate with FT management/GMICPmanagement/Commissioner., You are encouraged to maintain your Covid vaccination status unless medically exempt and will be supported to access information and guidance, to inform decisions that supports vaccine confidence, protects yourself and patients.

Provide leadership to Team Leaders to drive service improvements.

Undertake observations with Team Leaders to ensure exacting standards of practice.

Monitor performance of Team Leaders and support development as necessary.
