Deputy Team Leader - District Nurses

The post holder will be responsible for the planning, implementation and evaluation of nursing care needs and the delivery of high quality and up to date care to allocated patients in their own homes and/or a variety of settings.
The post holder will work within a team for a defined number of patients, and will work under the supervision of the Clinical Operations Manager. You will be responsible and accountable for the management of a Caseload in the absence of the Clinical Operations Manager/ District Nurse. You will also work closely with the aligned GP's to your team and will be both the advocate and the key chain of communication for their care., The person specification below is not the full person specification but outlines the criteria against which your application form will be assessed.
