Full Job Description

The post holder will be expected to be an effective clinical leader and working closely with the multidisciplinary team to identify new areas of potential development for nurse led services for patients with chronic disease.
Responsibilities will include the management and delivery of direct clinical care to patients, this will include; review, assessment, provision of education, advice, emotional and psychological support and counselling regarding range of treatment options. In additon the successful applicant will play an integral role in facilitating the Homecare Biologics care pathway and should possess the non-medical prescribing qualification (V300).
In undertaking this role, the post holder will ensure compliance with Trust clinical frameworks and essence of care, to ensure the delivery of the highest quality care to patients and support team members to embrace best practice and new models of working., To provide clinical and professional leadership and expertise in the speciality of dermatology, fostering a culture of continuous learning within each of the clinical areas.
Provides advice and guidance to other external healthcare providers, GP's, Community services and other healthcare professionals (advice line)
in consultation with the Consultant Team, develop, initiate and facilitate
clinical care pathways for patients suffering with Chronic Disease,
establishing and promoting clinical excellence through using evidence based practice.
Following the development of Chronic Disease services, ensure that staff personal development portfolios are maintained and that appropriate advice is given to the Matron and the Divisional Management Team to develop the Divisions Training Plan.
Working with the Matron, Divisional HR support, to ensure that appropriate mechanisms for the recording, monitoring, and management of attendance re in place in accordance with the Trust's policies and procedures.
In line with the Chief Nursing Officer's 10 Key Roles and in accordance with the Trust Policy for Nurse Prescribing to prescribe from an agreed formulary.
Responsiblility for the establishment and maintenance of specialist nurse led follow up clinics, providing advice, education and support for patients presenting with Dermatological disease.

An exciting opportunity has become available in our Dermatology Department for a highly motivated and enthusiastic Band 7 Dermatology Clinical Nurse Specialist (Chronic Disease Management).
The successful candidate will be required to work independently within a broad occupational framework.
