Full Job Description

We act as "conveners" for the local system, working collaboratively with our cross sector partners to improve the health and wellbeing of our population by using data and community insights to:
+ prioritise prevention
+ drive integrated working arrangements
+ optimise the use of existing resources
+ proactively support people at the greatest risk of the poorest outcomes
+ build resilient communities
Working to the Head of Place Development, Partnerships and Planning, your key areas of activity will include:
+ supporting programmes of work linked to the Place Board and the Health & Wellbeing Partnership(s) with a focus on addressing local priorities and delivery of the ambitions in the Joint Forward Plan
+ working with local system partners on the development of Integrated Neighbourhood Teams
+ supporting local delivery of the ICS Health Inequalities Strategic Framework for Action
We have three posts, working in North Norfolk, West Norfolk and Norwich. The posts involve hybrid working, based at home, attending your team office and travelling around the area to meet with colleagues and partners.

Working for our organisation
Health and care services in Norfolk and Waveney are working closely together to further improve services and provide more joined-up care for local people. In Norfolk and Waveney, we have already achieved a lot by working in partnership to improve health and care outcomes. These changes have been made possible by different organisations - NHS hospitals, GPs, mental health and community health services, local councils, care homes and social workers, voluntary and community organisations and others - joining forces to agree and plan for local people's needs.
Norfolk and Waveney Integrated Care System (ICS) includes a statutory Integrated Care Partnership (ICP), and an Integrated Care Board (ICB) called NHS Norfolk and Waveney. This partnership and organisation dedicated to making sure that organisations work together for the benefit of our residents, staff and communities is an important step change, helping to create positive differences to local people and joining up health and social care. This is the culmination of many years of effort to build partnership working across the NHS, local authorities, the third sector and patient groups., · The Development and Partnerships Manager will support the Head of Place Development, Partnerships and Planning (HOPs) to build and utilise relationships with key partners, who are represented at the Place Board, to foster a collaborative approach to deliver integrated models of care and service provision for local people. Work with other directorates in the ICB and alongside the D&P Managers and HOPs in the five places to support equity in service developments to patients and partners, and in the continued development of the N & W Integrated Care System (ICS).
· Support the Development and Partnerships team to ensure that work stream and programmes are planned and carried out effectively.
· Work closely with the Place Team.
· Support the Head of Place Development, Partnerships and Planning in cross-partner development which stimulates and further progresses system wide changes through an integrated approach.
· Working closely with Health and Wellbeing Partnerships across both local authority areas to support the delivery of work programmes.
· Develop an understanding integrated arrangements that enable cross boundary working.
· Work as directed with other directorates to realise clinical improvements and financial efficiencies where opportunities are identified. Support development and delivery of programmes of work at a Place level based on local population need.
· Be an advocate for the Place, its needs and population.

· Support the Head of Place Development, Partnerships and Planning to construct and deliver the Board agenda.
· Work with the Head of Place Development, Partnerships and Planning to deliver a local programme of work which will:
o Lead delivery in Place
o Ensure improvement is informed by the patient and public voice.
o Be informed by the health and care needs of the local population.
o Meet all statutory, regulatory and NHS requirements ensuring alignment with the activity of the organisation.
o Work with the Programme Boards and associated Integrated Care System (ICS) activities.
o Demonstrate and support a culture which promotes flexible arrangements through focusing on the outcomes for patients and systems and enables partners to understand their roles, responsibilities and contribution to patient and system outcomes.
o Be both progressive and responsive to place and system priorities, including work which impacts on health inequalities and prioritises prevention.
To be agreed but will be required to travel within and outside of the ICB's geographical area.
1. Key Working Relationships
· The post holder will be required to maintain constructive relationships with a broad range of internal and external stakeholders.
· Participate in relevant internal and external working groups/projects, services and initiatives to provide, information and analytical advice and expertise.
· Develop and implement data collection systems that will provide accurate and timely data.
· Present information and issues, explaining highly complex issues, to a wide range of internal and external stakeholders.
· To liaise with other managers to share best practice.
To work closely with the VCSE Network Lead to support delivery at Place and the wider VCSE integration agenda.

Hybrid Working
The ICB operates a flexible approach to working. It is expected the majority of our staff will work from a combination of home and office (where possible). For staff designated in a hybrid role they should have a safe and suitable place to work with Broadband access and space for a desk, chair and monitors. At interview a discussion will take place between the recruiting manager and the candidate to ensure they are clear on what is expected.

NHS Norfolk and Waveney Integrated Care Board (ICB) plans and buys healthcare services for our local population. We are accountable for the performance and finances of the NHS across Norfolk and Waveney - a total budget of £2 billion a year.
