Full Job Description

You will primarily care coordinate and be responsible for overseeing the recovery and care of a defined caseloads of patients under the guidance of the Team Leader and Responsible Clinician. You will work within a supportive established integrated multi-disciplinary team where we have strong values about working in a patient centred manner, where their recovery and wellbeing is at the heart of everything we do., You will provide a comprehensive accessible and responsive mental health service to Adults and their families within a defined locality.

To support the team and the Service Manager in implementation of the Dorset Healthcare Mental Health Strategy.

To continually develop clinical, team and service evaluation and delivery of service.

To contribute to effective multi-disciplinary working within the community mental health service and to build up effective liaison with community, primary, and other secondary and tertiary services/agencies within the locality to provide holistic care based on the individual's need and service development.

You can expect close working peer support and regular supervision to enable you to feel safe in your practice whilst further developing your skills.

Are you a reliable, committed, dynamic, and a motivated professional with excellent communication, organisational skills, and ability to prioritise work?, You should have recent experience of working in a mental health environment and be able to apply the philosophies of engagement and recovery in your work with patients and their families.

This role is full time based at Kings Park Hospital, Boscombe. You will need to have the ability to travel by car, with business insurance, as you will be required to assess and monitor patients in their own homes and within other community or inpatient settings.
