Full Job Description

The post holder will be a member of the diabetes team, providing ongoing education and advice to the person with diabetes and their family and to healthcare professionals involved with their care. The post holder will act as a role model and resource for staff that need specialist advice around diabetes management and supporting self-management. The post holder will work with the team to develop their practice in line with current best practice and National Guidance, and Innovative Practice Initiatives from NHS England., The post holder will deliver specialist care and education to patients with diabetes and those who care for them. They will be responsible for day-to-day management of their case load and for ensuring all care is delivered according to Trust policies and procedures while recording episodes of care accurately and in a timely manner following General Data Protection Regulation. They will be an integral part of the diabetes team in the delivery of up to date evidence based patient centered care, which is of the highest standard and demonstrates current best practice. They will provide specialist advice to patients, relatives and members of the multi-disciplinary team take part in clinical audit, research, and teaching. The post holder will provide an integrated link between GP and consultant led specialist teams and inpatient and community nursing teams including practice nurses community matrons and district nurses., An exciting opportunity has arisen for a keen and motivated nurse to join a well established community diabetes specialist nurse team in Surrey.
We are looking for a registered nurse who is passionate about providing high quality diabetes care, within the Guildford and Waverley community diabetes service,based at the Cedar Centre, Royal Surrey Foundation Trust.
You will be working within a supportive team of community diabetes specialist nurses and other health care professionals, to deliver clinical, education and psychological support to patients with Type 1,Type 2 and Type 3c diabetes, both within the hospital and community settings
The role includes working with GP surgeries, nursing and residential homes, providing specialist clinical support with insulin and GLP-1 initiation, insulin management, patients with more complex conditions and facilitating education to primary care colleagues.
The successful applicant will play a vital role when community diabetes patients are admitted to hospital, being involved in care planning, treatment/insulin dose and regime changes, discharge planning and follow up in the community.
You will need to have good time management, organisational and communication skills and be able to manage a clinical caseload.
