Full Job Description
The Cardiff and Vale UHB diabetes team are working with Value Based Healthcare to develop diabetes services. The expanded multi-disciplinary team will develop integrated pathways across primary and secondary care for adults with Type 2 Diabetes through the development of a community diabetes hub. The team will provide education and training to people with diabetes and community staff to support self-management and optimise glycaemic control.,
- Work with colleagues across primary and community care to provide support, treatment and education for patients with diabetes
- Enable an integrated pathway by acting as an interface between primary and secondary care, ensuring a system wide approach to managing diabetes.
- Be accessible to all client groups including nursing homes, housebound, custodial care and minority groups
- Have a positive impact across all clinical boards and diabetes clinical pathways, supporting admission prevention and improve flow through supportive discharge.
- This role is key to developing an enabling environment in order to provide patients and their families with the skills required to manage their diabetes.
We are looking for enthusiastic and highly motivated nurses to join our team. The successful candidates will need significant experience of working with people with diabetes and to have undertaken specialist post graduate education., The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
