Full Job Description

The post holder will provide support and education to patients with type 2 diabetes, with the support of a Highly Specialist Diabetes Dietitian. Developing new and innovative ways to support self-referral into structured education and the delivery of education groups.

To support the education team within the Integrated Diabetes Team Department; working as an integral member of the team to facilitate the therapeutic needs of patients with type 2 diabetes who are under the care of the diabetes team.

Manage patients with type 2 diabetes within an agreed scope of practice as allocated by the supervising Dietitian. Working without direct supervision and undertaking a variety of delegated clinical and non-clinical tasks, in accordance with agreed guidance/care pathways.

Provide a high standard of health care by assisting in and carrying out a range of individual and group education programs, in the community setting, for patients with type 2 diabetes, for which an agreed level of competency has been demonstrated under the direction of a Registered Dietitian.

Update resources for education programs and source new materials as needed.

Support and develop pathways for patient self-referral utilising appropriate communication channels, networking with local community groups and developing relationships with GP practices and local pharmacies.
