Full Job Description

We are looking for an energetic, highly motivated Lay Educator to join our Diabetes Team. We need someone who is enthusiastic and motivated who would like to provide high quality education for people with diabetes and those at risk of diabetes. You must have experience in working with people with diabetes or another long-term condition., You will work as part of the Diabetes Team in both primary and secondary care. You will organise, plan and contribute to the evidence based structured X-PERT group education for adult patients with Diabetes (Type 2) and X-POD Prevention of Diabetes programme, leading to an improvement in patients' abilities to self-manage their condition. Working alongside other members of the Diabetes Team as well as community groups, and other agencies to achieve this Other key duties:

  • To facilitate groups and assist in the delivery of structure X-PERT group education for adult patients with Diabetes (Type 2) , X-POD Diabetes Prevention Programme and EDDI our local structured group education for Newly diagnosed Type 2 Diabetes.
  • To assist in the provision of diabetes awareness events for City and Hackney.
  • To liaise closely with defined groups of primary health care staff based in City and Hackney CCG, ensuring education programmes are available in the community as well as in the hospital.
  • To assist patients and their carers to achieve individual goals to maintain physical and psychological well-being.
  • To promote physical well-being by delivering and encouraging patients to participate in the walking groups and gardening clubs available.

    You will be required to support the diabetes team with non-clinical tasks. You should also have proven teaching skills and leadership abilities. Fluency in another language, apart from English, would be an advantage.
