Full Job Description

The post holder will be responsible for providing; supporting and developing the delivery of high quality; evidence based Diabetes clinical practice throughout the local community, in health centre clinics and patients' homes. The successful applicant will be required to work closely with the lead Diabetes consultant, GPs, nursing colleagues and other members of the multidisciplinary team. The post holder will be expected to support their team, department and organisation to achieve the Trust's Values in their day to day work. These are the 5P's:

    We are looking for an enthusiastic and motivated individual, with excellent communication skills, who is able to work autonomously, as part of the team and to be an integral member of the wider Diabetes teams in supporting people managing the complexities of diabetes management.
  • This role requires facilitating patient structured education , having a sound knowledge of the principles of diabetes management and care, and a keen interest in professional development & advancing with technology. Therefore experience of teaching and good IT skills is essential. The ability to travel effectively across the boroughs is necessary.

    Starting with NELFTNELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

    This post also attracts payment for High Cost Area Supplement of 5% of the basic salary (with a minimum of £1,258 to a maximum of £2,122 p.a. pro rata for part time)., £46,148 to £52,809 a year p.a. (plus HCAS) pro rata
