Full Job Description

The primary aim of this role is to lead and develop the specialist nursing service support to primary care and support implementation of the NW London Diabetes Transformation Programme and to offer clinical, professional, and strategic leadership. The position involves identifying opportunities for service growth and leading the development and redesign of community-based diabetes care, including implementing care pathways and to develop robust partnership working within primary and secondary care to ensure true integrated care across West London Borough. By leveraging advanced clinical expertise, the post holder will influence GPs and commissioners while advising the Strategy & Business Development Directorate. The goal is to enhance and transform diabetes care for individuals in North West and West London boroughs., Additionally, the post holder will offer expert advice and support to colleagues and patients to uphold clinical excellence, reduce emergency admissions and bed days, and promote patient self-care and quality of life. They will provide clinical leadership and vision to support the Diabetes Specialist Nurses and drive the development of an integrated community diabetes pathway.
