Full Job Description

+ Clinically lead and deliver a phased programme of work to support a pilot Diabetes Value Programme
+ Undertake a value based review of the type 2 diabetes pathway from primary care into the secondary care gateway - comprising prevention, target screening (lifestyle etc), early diagnostics, primary care interventions, referral management and repatriation back to primary care.
+ Implement a range of evidence based methods to promote local access to consistent standards of diabetes care.
+ Manage agreed service delivery indicators to a successful outcome; liaising with the Specialty Lead for diabetes to monitor performance., 1. Provide clinical leadership in the delivery of secondary care based diabetes and related long term conditions by:
· developing and maintaining clinical standards in line with best practice
· developing a competency based framework for practitioners working in diabetes and related areas
· critical analysis, appraisal and synthesis of current research outcomes and/or relevant literature in order to maintain expert levels of knowledge and practice
2. Manages a highly complex and specialist caseload independently. Working as an autonomous practitioner to plan & carry out assessment and treatment, demonstrating autonomous clinical judgments where expert opinion differs due to an ambiguous or absent evidence base.
3. Uses specialist knowledge and critical appraisal skills to make clinical judgments in complex case management including differential diagnosis and formulates diabetes care planning based on best practice.
4. Continuously evaluates effectiveness and outcomes of clinical and educational interventions and makes recommendations re practice changes / service developments working with the wider multi-disciplinary team.
