Full Job Description

We are looking for an enthusiastic, experienced Diabetes Specialist Nurse with a particular interest in diabetes technology and patient education., The main activities of your role as an experienced diabetes specialist nurse will comprise of outpatient clinic reviews, supporting patients with diabetes technologies (e.g. Insulin pumps and CGM), answering patient queries through our diabetes helpline and delivering diabetes education. The post will also include reviewing patients admitted to the hospital with diabetes to ensure that their care and safety is maintained whilst they are an in-patient. The role includes liaising with other HCP's both specialist and non-specialist with-in the trust and with HCP's in primary care to ensure patient safety both whilst an inpatient and on discharge.

o To work as an autonomous practitioner and effectively co-ordinate and deliver an expert nursing service within diabetes

o To manage patients/client caseloads

o To ensure that patients and their families are fully informed and supported by providing clinical and social support

o To be involved in the development of evidence based clinical nursing practice in line with current research and guidelines

o To work collaboratively with colleagues at Princess Royal University Hospital, Intermediate Diabetes Nursing Teams in the Community, Nursing and other HCPs at Denmark Hill, KCH.

o To be involved in research/audit

The successful applicant must have undertaken a recognised programme of diabetes training and have demonstrable experience of working as a diabetes specialist nurse with proven clinical skills. You must be self-motivating, organised, computer literate and able to demonstrate excellent interpersonal skills with a commitment to continued personal development.

The successful candidate would be working within the skilled team of diabetes specialist nurses and specialist Doctors based at Denmark Hill. The Diabetes Centre at King's is at the forefront of advances in diabetes care and provides a number of specialist services which would also give the successful candidate opportunities to develop further skills as a specialist nurse., King's College Hospital NHS Foundation Trust is one of the UK's largest and busiest teaching Trusts with a turnover of £1 billion, 1.5 million patient contacts a year and around 15,000 staff based across 5 main sites in South East London.
