Join our dynamic team at CSH Surrey as a full-time, permanent Band 6 Diabetes Specialist Nurse and make a significant impact on the lives of those living with diabetes., We are seeking a dedicated professional to provide exceptional care to individuals with type 1 and complex type 2 diabetes across North West Surrey. In this role, you will collaborate within a multi-professional team, extending your expertise from the community to primary and secondary care settings.   • Conduct advanced clinical assessments and create personalized management plans for your patients.
  • Interpret clinical signs, laboratory results, and other functional measures to initiate appropriate treatments and interventions.
  • Make autonomous, safe, and effective decisions to enhance patient health outcomes.
  • Communicate complex and sensitive information to patients, their families, and carers with empathy and clarity.
  • Organize and prioritize your workload, with flexibility to meet service needs and uphold quality standards.
  • Lead and mentor other healthcare professionals, enhancing their skills and contributing to community and primary care development.
  • Drive the adoption of evidence-based clinical practices and develop specialist pathways within the service.
