Full Job Description

The services provide complex care to patients with diabetes within the community - in clinics and homes. You will also be involved in delivering structured education to patients to help them improve self- management of their condition. You will be supporting GP practices by telephone/email and through mentoring and participating in case reviews or virtual clinics, face to face clinics, held with GPs, Diabetes Consultants and other HCPs. As well as the clinical delivery, you may be required to lead on various service development projects such as audits, patient stories and patient engagement. In addition, you will be contributing to upskilling other health care professionals to provide a high standard of diabetes care.

We are looking for a diabetes nurse who is passionate about diabetes care and want to share their experiences. This role offers the opportunity for nurses with experience and skills in managing diabetes to develop their career in diabetes nursing., We are looking for a nurse with reasonable experience of Diabetes care and passionate about working with adult patients with Diabetes in the Community. The post-holders will have excellent communication and organisational skills. We actively encourage innovation and are committed to professional development.
