Full Job Description

  • To lead, develop, contribute to and influence strategies within host Trust, regionally and nationally to enable partnership development across Diabetes services.
  • Contributes to the provision of specialist nursing for Diabetes patients, practicing within the multi-disciplinary team.
  • Work collaboratively with partner Clinical Commissioning Groups (Integrated Care Services) to deliver education, training and raising awareness to General Practitioners, Acute and Primary Care personnel.
  • Improve the quality of care for patients and manage their own patient caseload with consultant supervision.
  • Responsible for providing advice, education and support to Diabetes patients, their carers and other health care professionals.
  • Deliver Integrated hospital / community specialist advice and support to patients, carers and other professionals about Diabetes.
  • To work autonomously in the assessment and development of clinical management plans and communicate recommendations verbally and in writing to Consultants, GPs, and other health professionals.
  • In collaboration with multidisciplinary colleagues within TGICFT and the community to take specific responsibility for the development of pathways of care and service redesign.
  • To lead the development of nurse led treatment and follow up clinics for patients that require active follow up.

  • Tameside & Glossop Integrated Care NHS Foundation Trust are looking for a self motivated and dynamic Band 7 Diabetes Specialist Nurse with extensive post registration experience to join the Integrated Diabetes Service Applicants must be able to demonstrate experience of managing Diabetes as a primary condition with a sound underpinning clinical knowledge of Diabetes management and related therapies. The service covers both Acute and Primary sites and involves working in a variety of settings affording the successful candidate opportunities to work with a range of patient groups. You will work as an autonomous practitioner providing a high quality, efficient and effective clinical service, delivering high standard of assessment, diagnosis, treatment and highly specialist expert advice to patients with Diabetes and their carers.
