To support transitional patients ensuring innovation is maximised in promoting engagement

An opportunity has arisen for an enthusiastic, motivated and experienced nurse with excellent communication to join the Diabetes Team. We are recruiting to a part time diabetes transition specialist nurse working closely with age groups between 16 and 25 years., Based between Bentley Medical Centre and Manor hospital you must be able to work autonomously supporting this population across the borough of Walsall , organize your workload effectively and adapt to, as well as drive change at a local level, being flexible is key. You should be a registered nurse or desirable registered children's nurse with extensive post registration experience in nursing. A post graduate qualification in diabetes or experience in diabetes clinical care would be preferred., Candidates should be aware that, regardless of country of origin, their ability to communicate in written and spoken English to the standard required to carry out the post, will be assessed during the selection process.
