Full Job Description

This post has been created utilising investment from Calderdale Integrated Care Board (ICB) to improve the uptake of annual diabetic foot screening in Calderdale. The project aims to improve patient compliance in Calderdale to bring it in line with the national average of 70%, and improve clinical outcomes for patients through supporting access to the necessary services. The post holder will lead on work to analyse the diabetic foot screening service in Calderdale, identify current limitations and implement strategies to optimise service delivery into the future. The post holder will support the Podiatry Team Lead for Calderdale and Huddersfield NHS Trust, and work in partnership with multiple stakeholders including Calderdale Primary Care Networks (PCN), General Practitioners, Practice managers and specialist diabetes nurses to establish initiatives across these organisations to improve access and optimise compliance with diabetic foot screening in Calderdale.,

  • To work with a range of stakeholders across the ICB who have responsibility for improving the experience and clinical outcomes for diabetic patients.
  • To work in collaboration with the Outpatient Therapy services manager and Podiatry Team lead to co-ordinate, design and deliver the project specifically on the provision and optimisation of diabetic foot screening in Calderdale.
  • To identify the current barriers to accessing diabetic foot screening for Calderdale patients.
  • To work with PCNs on improving planning and optimisation of the service.
  • To establish a model for year-on-year diabetic foot screening service planning to ensure long term compliance above national average in Calderdale.
  • To establish the scale of health inequalities in compliance with foot checks and implement strategies to address health inequalities where they are identified.

    This post will work in the Community Division at CHFT which predominantly provides support to the Calderdale area. The Community Division is a forward looking team which engages in developing services in the Health economy.
