Full Job Description

The successful candidate will work closely alongside our team of community dietitians to support the delivery of high-quality patient care in the community service. The role will involve providing clinical and administrative support to a number of different clinical areas across the service including tube fed children in Liverpool and Sefton., + To assist the specialist dietitians and other health care staff in providing a quality dietetic service to patients within the dietetic service.
+ To deliver specific elements of dietetic management within a clear defined scope of practice and with clinical supervision provided by the specialist dietitians.
+ To directly support families and other agencies such as schools/care settings to facilitate challenging dietary change.
+ To undertake administrative and clerical duties needed to maintain good organisation of the service and support the dietetic team.
+ Nutritional assessment and provision of dietetic advice, working alongside the dietitian to ensure that patient's nutritional needs are being met.
+ Use of a computerised dietary nutritional assessment program
+ Nutritional resource development for children, adolescents and families.
+ Service development, implementation and audit/survey of patient care practices as part of clinical governance., + To independently carry out patient assessments as designated by a Dietitian as part of clinical pathways.
+ To provide support and assistance to the Dietitian in the assessment and treatment of complex patients.
+ To assess patient's nutritional progress and provide advice as required under the supervision of the Dietitian
+ Use of computerised dietary nutritional assessment programme.
+ Regularly discuss with the specialist dietitian input/advice provided to families and any ongoing concerns. Safeguarding concerns to be raised immediately with supervising dietitian or head of department.
+ Attend multidisciplinary team meetings to discuss the management of individual patients, contribute to the on-going dietetic care and be involved in relevant strategies such as EHAT and Child in Need, to support families when indicated by social circumstances.
+ Assist with the development of practical, written and other educational resources to support the information given to families.
+ Liaise with GP surgeries regarding the need for prescriptions for specialist products.
+ Liaise with nurseries, school and other child care providers regarding provision of appropriate foods to meet specific dietary needs.
+ Interpret data from dietary analysis and compare with dietetic care plan. Identify and highlight any concerns.
Organisational
+ ICT & administrative support for the Dietitians in their specific area
+ To have good time keeping skills and working to deadlines
+ Prioritise own workload balancing demands of clinical care and other professional activities.
+ Maintain focus on clinical priorities of workload despite frequent interruptions to concentration by other referrals, phone calls and numerous other demands on time.
+ Support new departmental staff and students on clinical placement.
+ Record and submit clinical activity data according to departmental standards.

An exciting opportunity has arisen for an enthusiastic, highly motivated and organised individual to join our friendly community dietetic team., We are looking for an individual who has a passion for working in the NHS and a desire to improve patient care. You will be expected to have a foundation degree in nutrition or health-based subject or equivalent job experience. Ideally you should be able to demonstrate previous experience working in a health care setting and experience of working with multi- disciplinary teams and families would be an advantage. Candidates should be able to demonstrate that they can work effectively without direct supervision.
