Full Job Description
We have exciting vacancy for a 0.8 WTE Senior Dietitian specialising in diabetes and cardiac rehabilitation. This combined dietetic role will provide dietetic support for patients with Type 1 and Type 2 Diabetes seen in the Richmond Diabetes Service in addition to supporting patients referred into the Cardiac Rehabilitation Service. The role also includes providing nutrition support to patients on the inpatient unit at Teddington Memorial Hospital., The Richmond Diabetes service is a multidisciplinary service commissioned to provide a specialist integrated diabetes service to adults with Type 1 and Type 2 diabetes. The role involves outpatient support for patients with diabetes and structured education for Type 2 Diabetes. The candidate should be able to demonstrate advanced knowledge and experience of diabetes clinical management.
The Cardiac rehab role involves working within the multidisciplinary cardiac rehab team supporting this group of patients in specialist clinics and patient education sessions.
Main Duties:
To work as a member of the Diabetes Team, providing a comprehensive diabetes dietetic service for patients with Type 1 and Type 2 Diabetes.
Provide Dietetic support for patients with cardiac conditions known to the cardiac rehab service
Provide comprehensive dietetic advice to inpatients aiming to improve nutritional status.
Undertake comprehensive specialist assessments, set patient centred goals, create treatment plans, monitor and evaluate a designated caseload of patients, using evidence based clinical pathways.
Formulate treatment plans and goals in partnership with the patient, clearly communicate your clinical reasoning to justify your recommendations utilising the most appropriate treatment/intervention.
To provide specialist clinical advice, support and educate patients and carers in the management of their condition.
This is a great opportunity to develop your dietetic skills, knowledge and experience within diabetes, cardiovascular disease and nutrition support. You will need to be a confident, autonomous practitioner working in the outpatient setting however you will have the support of the multidisciplinary teams, and the dietetic department based at Kingston Hospital. This job will require some travel between sites.
