To address the ever-increasing demand for healthcare services and to better support individuals within their own communities, there is wide recognition of the need to embrace new ways of working and digital healthcare technologies to ensure all patients receive the right care, in the right place, at the right time.

We are looking for someone to join our existing team working 22.5 hours per week at our PCN base within Ludlow Youth Centre. Driving licence and access to a car is essential for the role., The role of the Digital Care Coordinator will be to support patients and healthcare professionals to maximize the potential presented by innovations in digital healthcare technology, and the associated new ways of working.

This is an innovative and exciting role, which is new to NHS organisations, and involves using available data sources to understand the health and well-being profiles of our residents. It means working within a team of PCN and practice staff to support their activities with the relevant data and data analysis, and it means helping to develop our digital maturity. But above all it means making a positive difference to the lives of our residents, by highlighting their needs and shining a light on hidden health inequalities.

South West Shropshire PCN is made up of 6 practices covering over 36000 patients over a large rural catchment area. The population is generally older than average, with a higher proportion of frail patients. Rural deprivation is a significant issue.

You will be employed on behalf of SWS PCN by Our Health Partnership.

Our Health Partnership was set up by local GPs who are passionate about providing high quality primary care and using their time and skills effectively to benefit patients.

We are currently a GP partnership of 30 practices with 39 sites, serving around 280,000 patients in Birmingham, Wolverhampton, and Shropshire.

£25,147 to £27,596 a year Pro rata. Depending on experience
