Full Job Description

The Digital Care Hub Manager is responsible for the provision of leadership, management, direction, and day to day financial control for all services delivered within the Digital Care Hub. The post holder is responsible for ensuring the operational delivery of the strategic and operational objectives for the services in line with National, System, Place and Trust initiatives.
The post holder will work closely with the senior managers and clinical leads in the Trust, Immedicare, ICB, Place, NHSE and other organisations external to the Trust to ensure delivery of high quality, safe, patient focused and cost-effective services in line with the Trust strategy and objectives. The delivery of these services involves health and social care collaboration as well as commercial partnerships.
Furthermore, there are excellent opportunities to work alongside Airedales new hospital programme, helping shape future digital services for the trust and the wider local area., Working in conjunction with the Head of Digital Care Hub to engage with new and current customers to develop and operationally deliver contracts and services, ensuring excellent customer experience.
The post holder will be required to create and manage long-term relationships between the Digital Care Hub and a range of clients (NHS, local authority, voluntary sector, commercial companies).
Overall management of services delivered from the Digital Care Hub which at present include:
· Immedicare - National care home service
· Goldline- Palliative Care service
· MyCare24- Long term Conditions/Virtual Wards
The post holder will work with the local providers of all clinical services as well as customers to define and maintain standards of service delivery and develop the service in a way that aids client retention and generates new business. The post holder will have day to day budget responsibility for all services delivered from the Digital Care Hub. All staff in the Digital Care Hub will be operationally accountable to the post holder - clinical staff will be professionally accountable to the Nurse Consultant, Digital and Acute Care., 1. Overall staff management within the Digital Care Hub to include manpower planning, recruitment, career development, performance management and work evaluation.
2. Empower and enable all staff within the services/teams to fulfil their duties and responsibilities and support the continued development of their roles.
3. Ensure all staff have an annual appraisal with agreed objectives.
4. Establish clear lines of communication between managed areas, other agencies and statutory bodies providing coordinated and planned partnership working.
5. Responsible for the operational delivery of agreed services.
6. Responsible for ensuring the management of all patient records in line with Trust policies for recording, filing, and storing of confidential information.
7. Ensure security of highly confidential patient information in line with contractual agreements.
8. Information Asset Administrator for all electronic systems in the Integrated Care Hub.
9. Collaborate with the Head of Digital Care hub and Nurse Consultant on policy development and implementation for Digital Care Hub Services.
Service Development
10. Work in collaboration with the Head of the Digital Care Hub and Clinical Lead, Nurse Consultant for digital and acute care to enter into negotiations with potential and current customers to shape, develop and define services with all clients.
11. Engage with clients to demonstrate and identify alternative solutions for the delivery of traditional services in line with Trust and national strategies for new models of care which will include representatives from health, social care and the voluntary sector.
12. Oversee and develop training packages and standard operating procedures in partnership with Immedicare, the Trust and clients to meet customer needs and ensure delivery of safe, high-quality services.
13. Manage and forward plan new business and services, including capacity modelling and workforce development in conjunction with the Head of Digital Care Hub.
14. Maintain the high profile of the Digital Care Hub to support the spread and adoption of technology in the delivery of mainstream services.
15. Be an active member of the Senior Management Team for Immedicare to drive the joint venture forward operationally.
16. Foster strong relationships with the Immedicare team at all levels
17. Co-chair the Immedicare Operational Group meetings in partnership with the Contract Performance Manager for Immedicare
18. Identify risks, anticipate issues, create solutions and resolve problems in relation to project and/or service delivery.
19. Ensure quality and safety is at the forefront of all services delivered from the Digital Care Hub.
20. Investigate and respond to complaints/comments and general feedback from health professionals, patients, families and carers.
21. Ensure all Adverse Events are fully investigated and closed in a timely manner.
22. In response to complaints/incidents, identify and implement corrective action and learning to avoid recurrence wherever possible.
23. Deliver national and local training and present complex digital healthcare information at Place, ICB and NHSE levels as well as to Care Homes, Community Teams, GPs (this list is not exhaustive).
24. Maintain an up-to-date knowledge of national, local and Trust strategies and latest industry news.
Contracting and Finance
25. Responsible for day-to-day management of the service budgets across the Digital Care Hub.
26. Work with the Head of Digital Care Hub to develop financial models for new and existing services.
27. Attend Directorate Performance Meetings to present KPI and Performance information.
28. Develop business cases for enhancing existing services and developing new services.
29. Work with Information Services to ensure that reporting is developed and delivered to meet contractual KPIs and SLAs for customers.
30. Work with Information Services to develop reporting and intelligence to understand the performance and impact of the Digital Care Hub services.
31. Ensure compliance with Service Level Agreements.
32. Maintain a working knowledge of Trust and NHS procurement policies.
Any other duties necessary for the successful performance of the
role.
