Full Job Description
A key initial responsibility will be embedding and expanding the rollout of a new primary care system across Hillingdon. The successful candidate will ensure this system delivers maximum benefits for GP practices, their patients, and other local organisations providing primary care services.
We are seeking a skilled and motivated Digital & Transformation Lead to support the delivery of digital services and provide technical support across The Confederation Hillingdon CIC, Hillingdon PCNs, and GP practices. This role will focus on improving processes, leading learning and development initiatives, and driving digital innovation across primary care in Hillingdon. The ideal candidate will have expertise in primary care change management, including conducting needs analyses and managing projects to meet tight deadlines. They will create project documentation, monitor progress, and provide insights to ensure goals are met. Strong communication skills are essential, as this role requires close collaboration with practice teams, including GPs, key suppliers, patients, and other internal and external stakeholders. Candidates should possess a strong background in health systems, preferably within NHS primary care, and a proven track record of delivering results under pressure. This role is ideal for someone passionate about digital transformation and making a positive impact in healthcare and for patients.
The Confederation, Hillingdon CIC works with general practice and other healthcare providers in Hillingdon to deliver high quality clinical services to patients. Our aim is to improve care for patients by working collaboratively across primary care and our partners as part of the Integrated Care Partnership. The Confederation team also work to develop and support individual GP practices, PCNs and Neighbourhoods and their changing needs. We are of the NHS but independent, innovative and transformational.
