Full Job Description

You will work alongside the ward teams to provide support and advice on discharge. This role includes the Co-ordination and completion of referrals to other services, working to tight deadlines and communicating with patients, families and the wider MDT. Being a point of contact with both the ward team, patients and families is vital to help reduce the length of stay for patientsMain duties of the job To be responsible for the coordination of the ward's discharge processes. To ensure patients, families and carers are involved in the discharge process from initial admission to actual discharge. Communicates regularly with patients and their relatives to promote and maintain understanding of discharge planning and discharge progress. Is the focal point of contact for communication regarding discharge information and planning for families, patients and MDT. Liaising with other agencies both internal and external to the trust to provide a holistic approach to discharge. Ensure relevant processes are followed to enable timely access to social care, rehab facilities and all community services. Early identification of potential problems that may affect discharge and the development of initial plans to address the issues ensuring escalation to relevant teams As an employer and sponsor licence holder, to be able to provide sponsorship to overseas nationals via the Skilled Workerroute we must ensure that we adhere to the sponsorship requirements set by UK Visa's and Immigration.
