Full Job Description
We are searching for a full time (37.5 hours) Band 5 Discharge Facilitator to join us on Beech Ward based in Torquay, Torbay Hospital. Beech ward is a 16 bedded inpatient Mental Health Ward that supports primarily older adults with functional mental health difficulties. The successful candidate will be supported by a team of experienced professionals, including a variety of multi-disciplinary team members, Band 6's and Ward Manager. Working alongside the Nursing Team, and within the MDT, you will ensure that service users receive a high standard of person-centred care which enables them to recover in a timely way., Beech Ward provides acute inpatient care to older adults experiencing mental health difficulties. The role of Discharge Facilitator has been developed to specifically work with inpatient and community colleagues as well as family members/carers and service providers to support smooth discharge pathways out of hospital. Using care management techniques and a multi-disciplinary recovery oriented approach the post holder will be expected to support individuals to access community resources and local provision to meet their needs in addition to brokering appropriate funded move on placements or support packages. Develop working relationships with service users, carers/family members to facilitate discharge from hospital Establish firm working relationships with inpatient, community mental health team colleagues, and housing and support providers from public, private and third sectors. Develop and accurately record comprehensive support plans, risk assessments and relevant reports to inform care management and social care processes. Represent the Trust at meetings in an appropriate and knowledgeable manner including, on occasions, giving presentations using relevant media. Communicate highly complex and sensitive information effectively across all levels within and outside of the organisation, to advocate collaborative working and promote best outcomes. - Knowledge and understanding of the legislative framework around mental health, social care and housing, eg: Care Act Mental Health Act, Mental Capacity Act., Educated to degree level or equivalent in: Health/Social Care or equivalent housing qualification or experience
- Registration with Professional Body if qualified healthcare professional., Experience of care management and working with people with mental health difficulties
- Experience of working with people with mental health difficulties in a community and inpatient setting
- Experience of multi-disciplinary/multi-agency working, particularly regarding housing, accommodation and support providers, Knowledge and understanding of the legislative framework and direction of mental health, social care and housing, eg: Care Act, Mental Health Act, Mental Capacity Act.
- Must have good understanding of the social care needs of people with mental health problems
- Must have knowledge of and work in a recovery orientated approach with people who use mental health services, Experience of care management and working with people with mental health difficulties
- Experience of working with people with mental health difficulties in a community and inpatient setting
- Experience of multi-disciplinary/multi-agency working, particularly regarding housing, accommodation and support providers
- Excellent communication skills, being able to negotiate, challenge and manage conflict
- Ability to create effective working relationships with colleagues, service users, carers and partner agencies
- Ability to use IT and recording systems as per policies.
- Ability to report and present information
- Ability to represent the Trust in a professional capacity and work within the Trusts values
- Ability to assess need, risk and make decisions in order to support service user discharge from hospital.
- Ability to analyse information and data to extract pertinent facts.
- Ability to exercise individual judgement and consult where and when appropriate
- Able to plan and organise and prioritise their own work schedule and deal with urgent situations
- Ability to be flexible, creative and use own initiative
- Ability to be mobile across Trust locations

- An Enhanced CRB Check will be required for this role., Ability to work effectively in a team
