Full Job Description

The Specialist Autism Team was first establishedin May 2021. Sincethenthe team has continued to grow and develop. The service is commissioned to offer anInreach, Outreach and Discharge Facilitation.

We want to hear from you if you are highly committed to working withautisticadults.We are offering anopportunity for a registered nurse(Mental Health or Learning Disability),AHPor social worker to join our multi-disciplinary teamas part of our discharge facilitation pathway.

Within this pathway, ourprimaryroleis to supportautistic individualsback into the community, planning discharge collaboratively from the point of admission ina timely, wellplannedand sustainableapproach.You will also support the individualand their support networksfor 6 weeks post dischargeto ensure their discharge is safe.

You will work closely with the Multi-disciplinary Team of the Specialist Autism Team which consists ofSpeech andLanguageTherapistsand Assistants, Occupational Therapists, LearningDisabilitiesNurses, Mental Health Nurses, PsychologyandAssistantPsychologyand widerMulti-disciplinaryteam such asLocalAuthority,KeyWorkersand the Integrated Care Board (ICB).

The Service strives to provideforward thinking high qualityservices, working in partnership with other agencies. In addition to working withautistic peopleand their families we have a strong commitment to public health, promotion of safeguarding and effective risk management through service development and training initiatives working within an enthusiastic and experienced team.

A RegisteredProfessional, withup to dateknowledge, of health policy and legislation applicable toautistic people.Adaptability,good communicationskills and self-motivation are essential. Anexperienced clinician with proven skills in working with people who have complex health needs and challenging behaviour andautism,demonstratingpractice is evidence based and innovative.

The post holder willbe responsible fora team caseload and supervision of other practitioners (registered and unregistered), to provide strong clinical leadership and supervision within theSpecialistAutism Team.

The post holder will work flexibly between the hours of 8am and 8pm by working a rotational shift pattern including weekend working to meet the needs of the team and the wider network.

Lancashire & South Cumbria NHS Foundation Trust provides a range of services including secondary mental health care across this area. Specialist provision comprises inpatient child and adolescent mental health services, perinatal mental health and forensic services including low and medium secure care.
