Full Job Description

An exciting opportunity has arisen for discharge keyworker to join UHD Discharge Team. The post holder will be situated in the hospital discharge team, designated to clinical areas to support discharge planning according to hospital flow pressures. The post holder may be required to participate in cross- site working between Poole and Bournemouth hospital sites. The position will be required to work across a 7 -day rota / the service operates on weekend and Bank holidays, The successful individual will spend the majority of their working day to:

  • Visit wards, advise and direct discharge planning / Discharge to Assess (D2A) process
  • Support hospital staff with assessment and decision-making.
  • Liaise between the ward staff, therapists, social workers and external partner's staff.
  • Guide staff to improve D2A referrals/ process.
  • Support effective discharge planning reporting via the Health of the Ward IT application and other IT applications associated with patient information.

    It is essential the successful candidate is able to be adaptable to different working priorities as the clinical area designated will change according to the hospital pressures been experienced on a daily basis.
  • The post holder will have well developed interpersonal skill as the role requires the individual to communicate effectively at all levels, demonstrating negotiating, problem solving and solution creating skills.
