Integrated Discharge Team

An outstanding team needs outstanding members! This is a great opportunity to be a part of a supportive, caring, flexible and friendly team.

We are seeking an enthusiastic and motivated Registered Nurse to join our team in IDT to support the discharging of our End of Life Care patients via the Fast Track referral process.

The post is Monday to Friday with some weekend working to cover the hospital for all IDT patients.

The successful candidate will need to be a registered nurse with active NMC registration, an interest in End of Life Care would be desirable but not essential. Evidence of excellent verbal and written communication skills. An ability to work autonomously and demonstrate resilience.

We provide emergency and non-emergency services to the residents of East Surrey, North-East West Sussex, and South Croydon, including the major towns of Crawley, Horsham, Reigate and Redhill; a community of over 535,000 people. We provide acute and complex services and a range of outpatient, diagnostic services whilst Crawley Hospital and Horsham in West Sussex and Caterham Dene Hospital and Oxted Health Centre in Surrey provide less complex planned services.
