Do you have a passion for patient-centered care and planning patient discharges? Are you looking for a new challenge within a friendly supportive team?
We are looking for a new member of staff to join us at Tunbridge Wells Hospital, as a Discharge Liaison Nurse within the Transfer Care Hub.
The Transfer of Care Hub is an integrated multi-disciplinary team who work together to ensure timely and safe discharge linking hospital and community services to ensure the best outcome for people leaving hospital whether that be to home, a discharge to assess community bed or long-term placement.
You should have some knowledge of the discharge processes and pathways; have an understanding of how to complete Fast Track documentation, Ready to transfer forms, the Discharge to Assess process etc.

The department works closely with all members of the Multi-Disciplinary Team in the acute setting and Community. The Discharge team work to ensure we achieve the best outcomes for our patients and their relatives in facilitating a timely, efficient but safe discharge.

A Discharge liaison Nurse should be able to support the wards in organizing discharges, gaining relevant information from patients and relatives. Liaising with the MDT and completing necessary paperwork to make referrals to appropriate teams; including referrals made online. Computer literacy and an excellent telephone manor are required, also the ability to deal with confrontation in a sensitive manor. A sense of empathy and understanding of the qualities required to with patients including confidentiality and respect are also required.

Following your Corporate Induction you will be expected to attend the Trust's 2-week Induction Programme for RNs, NAs, RMs, ODPs and Paramedics. The programme is full-time (please email mtw-tr.professionalstandards@nhs.net if this is problematic) and will involve travel to both Trust sites. The programme includes supernumerary shifts in the clinical area, classroom-based sessions and e-learning.
