Full Job Description

An exciting opportunity has arisen within the Discharge Team. You will work as part of team co-ordinating referrals received from our wards and sending to the appropriate service following a validation process. You will helping to improve patient flow primarily working at Poole Hospital. The role will incorporate cross site working and cover with Bournemouth Hospital.
You will be working closely with our Partners and building good working relationships.
The role is full time and cover a 7-day working pattern.
