Full Job Description

You'll be supporting people who are discharged from hospital, making a direct difference to their lives by ensuring they receive the relevant aftercare and support. What you will be doing

  • As a primary purpose for this role, you'll be responsible for maintaining and updating information systems and databases to ensure that service transactions are completed accurately.
  • Daily, you'll provide advice to customers, contacts, external organisations, and colleagues ensuring that they receive appropriate guidance about policies, procedures and processes whilst ensuring effective working relationships are maintained.
  • It will be your responsibility to respond to queries, complaints, and requests from initial contact with the customer, issues that are not possible to resolve are promptly relayed to more senior members of your team.
  • You may be involved in carrying out research; collating data and presenting information within your area of service specialism. This may include drafting documentation and clear, accessible findings.
  • You will provide a wide and varied range of administrative tasks, including maintaining multiple spreadsheets to accurately record and monitor the work of the hospital team. You will ensure support is provided, and records are accurate which will help to inform decision making along with monitoring and maintaining budgets.

    We require you to have experience in a relevant area of statutory or voluntary sector work in the health or social care field.
  • You will be educated to a minimum of 3 grade C GCSE's (or equivalent) including English and Maths.
  • You'll possess a good understanding of working in the community in Social care or health setting with the ability to understand individual needs.
  • You will be asked to collate and distribute appropriately a range of information relating to service users.
  • It will be key that you have a good knowledge of Microsoft Excel and IT skills to use a client data system.
