Full Job Description

Our Occupational Therapy Team are looking for a D2A Coordinator who will be supporting the Senior OT (Integrated Therapies) and OT Service Manager with the delivery of referrals via the D2A (Discharge to Assess) pathway. The D2A Coordinator will coordinate referrals ensuring flow out of the hospital into Reablement and other discharge pathways as required. Annual increments, where applicable, are usually payable on 1st April each year, subject to six months service in the post, until the maximum spinal column point of the level has been reached.
