Full Job Description
An exciting opportunity has arisen for a Band 6 District Nurse / Case Manager.
We are looking for a District Nurse or a suitably experienced Community Nurse who can demonstrate a wide range of clinical and leadership skills to complement our now fully Integrated Care Community Team. Crewe is the largest Care Community and has 2 District Nurse Teams, although a Care Community approach is fostered promoting close working for both teams.
This post will be based with the Crewe DN Team based at Eaglebridge . You will be joining a hardworking and compassionate District Nurse Team, who work together to provide the best care for patients in the Crewe Care Community., As a District Nurse / Case Manager you will work in partnership with our patients and their families to lead in the delivery of person centred, safe and effective care. The successful candidate will require extensive experience in community nursing, have sound clinical knowledge and skills in the management of patients with complex needs, long term conditions and end of life care.
You will have a passion for quality patient care and be able to demonstrate ongoing clinical professional development relevant to this post. You will be supported to develop your leadership, organisational and communication skills and how to manage challenging situations in a professional and flexible manner.
