Full Job Description

We are looking for a District Nurse or a suitably experienced Community Nurse who can demonstrate a wide range of clinical and leadership skills to complement our now fully Integrated Care Community Team. Northwich is the second largest Care Community and has 2 District Nurse Teams although a Care Community approach is fostered promoting close working for both teams., As a District Nurse / Case Manager you will work in partnership with our patients and their families to lead in the delivery of person centred, safe and effective care. The successful candidate will require extensive experience in community nursing, have sound clinical knowledge and skills in the management of patients with complex needs, long term conditions and end of life care.
You will have a passion for quality patient care and be able to demonstrate ongoing clinical professional development relevant to this post. You will be supported to develop your leadership, organisational and communication skills and how to manage challenging situations in a professional and flexible manner.
Although this post is based in Northwich, there will be at times a requirement to work across the footprint of Central Cheshire Integrated Care Partnership. Travel is an essential part of this role; please demonstrate on your application form how you would meet this requirement., The post holder will be responsible for the management and coordination of patients requiring community nursing input. The District Nurse/ Case Manager will oversee/undertake the assessment of complex nursing care needs, the development, implementation and evaluation of programmes of individualised care to patients on a defined district nurse case load.
They will deputise as required in the absence of the team leader to ensure continued efficient and effective service delivery.
They will work as part of the integrated community health care team, working within and across professional and organisational boundaries covering the 24 hour period of service delivery.
They will assist and support the district nurse team leader in providing clinical leadership to the community nursing team and to assist in leading the team in the provision of high quality, evidence based programmes of care.

    This post will be based with the Victoria Infirmary DN Team based at Verdin House. You will be joining a hardworking and compassionate District Nurse Team, who work together to provide the best care for patients in the Northwich Care Community.
