As aDistrict NursingSisteryou will manage a clinical caseload under the delegation of the team leader as well as exercising leadership and management of a defined community DN team to ensure good clinical practices andhigh standardsof care aremaintainedin order tomeet service demand.We offer a supportive environment with opportunities for further career development,anda strong in-house training programme. Working within our Community Nursing Team you will support, assess, and treat patients with a variety of health needs, delivering high quality, evidence-based practice. You will have a passion and enthusiasm to care for patients in their own homes.

  • Maintain required competencies, asidentifiedin the Band 6 Community Nurse competency framework,in order toeffectively undertake patient assessment, care planning and immediate care needs.
  • Support staff to complete holistic assessment and reassessment of all health and nursing needs, ensuring that every patient has an overview of health and nursing need recorded.
  • You will manage a complex and diverse caseload of patients
  • Support teams to provide information to patients, theirfamilies,and carers both formal and informal, so that they can make informed choices about current and future care needs.
  • Provide clinical support, leadership, and guidance to all staff to ensure that evidence-based care and clinical practice aremaintainedin Community Nursing Teams.
