Full Job Description

  • To assess, plan, implement and provide regular evaluation of health care needs in conjunction with patients, their carers and other appropriate members of the multidisciplinary team.
  • Prioritisation, organisation and appropriate delegation of the responsibilities and workloads within the team will be required to ensure efficient and effective service provisions
  • To be responsible for the caseload management of patients within the practice population.
  • To be responsible for keeping up to date with current issues and events in Primary Health Care.
  • To actively participate and influence the developments of Community nursing services.
  • To provide support and health education programmes for patients and carers through effective interpersonal skills.
  • To work flexibly and relieve colleagues/ rotate within the trust as required and requested by the line manager to meet the needs of the service.
  • To work alongside other members of the multidisciplinary team e.g. attend meetings of Primary Health Care Team.
  • To ensure own mandatory training and team members mandatory training is kept up to date in line with Trust policies
  • Ability to use Information Technology including Word, email and the internet and RIO
  • Participation in complaint investigations, when requested.

    We are seeking to appoint an innovative Community District Nursing Sister/Charge Nurse. You must hold the District Nurse Specialist Practitioner Qualification. You will manage a District Nursing team within the Greenwich Directorate of Oxleas NHS Foundation Trust.
  • The successful candidates will be part of a progressive and supportive team located in South East London. Our District Nursing Service delivers care to a diverse population across the borough of Greenwich. We are looking for an enthusiastic, conscientious, adaptable manager and team player with highly developed nursing skills. You must be able to work on your own initiative. Previous clinical team management experience is essential. In our district nursing teams, due to the requirement to be able to travel between patients' homes in a timely manner, a full UK driver's licence is essential as well as access to a car for work purposes.
