St Ann's School is an outstanding secondary special school in Ealing with 117 students on roll who have complex disabilities, including profound and multiple learning and physical disabilities, severe learning difficulties, and complex autism. The Role

As a SEN Family Support Worker, you will contribute to the delivery of an 'outstanding' School Improvement Service within Ealing's Children & Families Directorate. Supporting the work of the SEND Consultants you will deliver interventions to different family members, help assess their needs and develop a whole family plan to achieve agreed outcomes that promote the safeguarding of children, the understanding of needs, attendance at school and positive citizenship.

The Role

As a SEN Family Support Worker, you will contribute to the delivery of an 'outstanding' School Improvement Service within Ealing's Children & Families Directorate. Supporting the work of the SEND Consultants you will deliver interventions to different family members, help assess their needs and develop a whole family plan to achieve agreed outcomes that promote the safeguarding of children, the understanding of needs, attendance at school and positive citizenship.

Key responsibilities and duties of the role include:

1. To engage families in recognising the need for change in order to improve outcomes for their children and work with them to achieve sustainable changes using a variety of professional skills under the guidance of the SEND Team.

2. To work purposefully with schools, health professionals, voluntary sector, police, social workers and other professionals to achieve best outcomes for children with SEND under the guidance of the SEND Team

3. To provide written and verbal reports that are able to demonstrate appropriate analysis of issues.

4. To provide a responsive, flexible outreach service to families in the borough under the guidance of the SEND Team.

5. To provide advice and support to families and professionals in the community in regards to resources for families, the early help and SEND support offer and signpost safeguarding concerns to management.

6. To contribute to the monitoring, evaluation and performance management of the service including obtaining feedback from parents, children and agency colleagues.
