Full Job Description

We have various positions available in our Drug and Alcohol Outreach, Hospital Outreach and Housing teams. Successful candidates will also work alongside in-house services which include community, clinical, criminal justice and a residential rehabilitation facility., Hospital Outreach Our Hospital Outreach role offers an exciting and challenging role that works from a trauma-informed perspective based in the community covering all of Cornwall. The service provides a rapid response to individuals who are frequent attendees at the Royal Cornwall Hospital in Truro and have issues related to substance use. The team works within a multi-disciplinary model of practice with partners in safeguarding, housing and homelessness services, domestic abuse services, as well as primary and secondary health care and mental health providers. Drug and Alcohol Outreach This role will be working alongside other services providing outreach support to clients around Cornwall with drug and alcohol issues who are struggling to maintain their housing. You will work alongside the Adult Community Service and the Young People's Service to ensure that an accessible treatment offer is available to those who find themselves rough sleeping or in insecure accommodation. You will be
a resilient and committed individual who has an understanding of the problems faced by individuals finding themselves homeless or in insecure accommodation. Floating Support Outreach As a Floating Support Outreach Worker, you will be trained in all specialisms mentioned above and will provide support to the teams, depending upon where the need is greatest.
