An exciting opportunity has arisen for highly motivated and passionate administrator to be part of a new team working to prevent unnecessary admissions to mental health hospitals of adults with a learning disability and/or autism.

As the Dynamic Support Pathway Administrator you will support the Team Co-Ordinator to manage the referral processes for individuals whom have been identified as having complex needs requiring additional support, work with professionals from (but not exclusive to) Health, Social Care, Education, Private Charitable Organisations etc to co-ordinate Multi Agency Meetings, undertake generic admin processes such as minuting meetings, responding to emails, supporting clinical professionals within the team and maintaining accurate records.

You will be experienced in working autonomously and as a member of a multi-disciplinary team keeping the person at the centre of everything you do. You will have excellent communication and interpersonal skills, be able to problem solve and demonstrate the Trust Values in everything you do.

You will join the award winning LDA Collaborative and will be part of making a difference and improving the lives of local people., The Dynamic Support Pathway Administrator is primarily a home based role however there will be opportunities to attend face-to-face meetings/training/development sessions.

The successful candidate will be part of a busy dynamic service with responsibility for arranging clinical and non-clinical meetings including notifying individuals e.g., parents, carers, staff, other professionals of meetings, managing changes, cancellations and any additional needs, often at short notice, as required.

  • To process patient referrals ensuring they meet Service specific protocols to assist with the quality of patient care.

  • To accurately type or transcribe from audio or paper, patient or other information to maintain records and /or produce correspondence to support service record keeping and communication.

  • To support the DSP Co-Ordinator and Officer in the course of their duties - including maintaining the Dynamic Support Register and Care and Treatment Review protocols.

  • To confidently communicate with the individuals referred to our service, their family/care team and other professionals across Health, Social Care and 3rd Party Organisations.
