Early Stroke Rehabilitation Team Physiotherapist

The Early Stroke Rehabilitation Team (ESRT) is a friendly, enthusiastic and supportive team who are passionate about delivering high quality patient-centred care., We have an exciting opportunity for a band 6 physiotherapist to join the team on a fixed-term basis (12 months). You will work alongside a team of physiotherapists, occupational therapists , speech and language therapists, clinical neuropsychologist, stroke nurse and rehabilitation assistants.,

• Working as part of a multi disciplinary team


• Undertaking assessment and treatment of patients with stroke in a community setting


• Plan and implement a comprehensive programme of intervention for patients with stroke, including those with a complex presentation/multi-pathologies


• Work alongside our partners across the stroke pathway for example the Stroke Clinical Specialist, other community services, and the acute hospitals



• To implement strategies to empower patients and families/carers to take a lead role in their rehabilitation and long term management of their condition



• To work collaboratively as part of an inter-disciplinary approach within ESRT and Neuro Rehab Service and Neuro Clinical Specialist Team by attending and contributing to multidisciplinary meetings, participation in patient discussions/treatment plans and discharge planning
