Full Job Description

Job Purpose/Summary To take responsibility for the day-to-day monitoring, organisation, and oversight of the PCN Enhanced Access Service, and other programme responsibilities as part of the Leadership and Delivery portfolio. This will include working with member practices to develop processes that ensure delivery of service requirements that meets the needs of our Primary care Network and ensures compliance with the Network Contract DES. To support the PCN Clinical Leadership & Development Manager to formulate the future roadmap and goal setting of the service to create a first-class service that supports demand and ensures patient experience. The role will require exceptional diary organisation skills, planning and flexibility to provide service cover when required, and to ensure that service team staff have access to required training, upskilling and development within their roles, including a programme of induction and familiarisation to the service, locations and systems used. Work
with the wider team to ensure that the service, patients, their relatives, carers and staff have a positive experience when accessing and receiving care/services from the members of the Primary Care Network, within the services that are delivered. Main Duties and Responsibilities Enhanced Access To work with the Clinical Leadership and Development Manager, Out of Hours Care Coordinator, Enhanced Access Clinicians, Nurses, and administrative staff, alongside the member practices to ensure service delivery, safety and effective utilisation of Clinical and non-clinical appointments within the service offering. This includes: 1) Ensure that sites from which the service will operate are available and compliant with regulation/CQC requirements. 2) Develop a workforce that meets the needs of the service and is appropriately qualified and trained in the way that the service works. 3) Liaise with member practices and third parties (where applicable) to develop a service provision that supports
member practices with access and capacity and supports the contractual requirements of the NHSE GP contract. 4) Day to day responsibility for rotas, clinician allocation to sessions, dealing with questions and queries from Practices, Clinicians, and Patients or their advocates as required. 5) Ensuring all Clinical and Digital systems are fully functioning and effective, and where issues arise liaising with the providers to ensure a rapid and effective resolution is identified. 6) Participating in audit schedules for utilisation of appointments available and presenting these in data formats. 7) Developing ideas and plans for development of the Enhanced Access Service in order to support Primary Care and the PCN Member practices further. 8) Maintain knowledge and understanding of the requirements of General Practice including QoF, IIF, Patient Satisfaction and Surveys, Capacity and Access and Winter pressures. Projects 1) Support the PCN Clinical Leadership & Development Manager, the PCN
Manager and the Digital and Transformation Lead with projects and service development work as required.
