Full Job Description

We are seeking to recruit a registered adult or registered enhanced mental health nurse to the post of enhanced care specialist practitioner to manage and support our team of health care support workers as part of our Enhanced Care Team who provide 1:1 support and meaningful activity/intervention for in-patients who have enhanced care needs. This includes patients who require specialist mental health support while receiving care in the acute general hospital environment.

This is a new and exciting post with the opportunity to be innovative and develop new practices with the support of the wider nursing and multi-disciplinary teams.

The post advertised is full time and a permanent position, but we are happy to consider applicants who would like part time hours as part of a job share, and staff who may wish to pursue this post as a 12 month secondment.

Main duties of the job

Operational day to day management responsibility at team level using performance management systems to ensure the effective delivery of activity.

Coordinate the daily allocation of Enhanced Care Team

Collaborative working with ward teams, contributing towards the assessment, review and oversight of patients requiring a higher enhanced level of care.

Take an active role in the risk assessment and planning of patient care in collaboration with the ward teams and supporting implementation of strategies to minimise risk.

Regular contact with with liaison psychiatry team to ensure safe care, risk management and care planning of patients who require specialist 1:1 mental health support., children, young people & vulnerable adults.
