Enhanced Care Team, Clinical Nurse Lead

We are recruiting for a Enhanced Care Team, Clinical Nurse Leadwho shares our ambition and vision to deliver theBest of Care by the Best Peopleand has the drive to take us forward, by ensuring that the Trust continues to be the first choice for patients and our staff., They will directly manage and lead the Dementia and Delirium Team, triaging referrals effectively and ensuring onward referral to an appropriate clinic or organisation, and evaluate the service for quarterly reports.

To liaise with community and ICB partners promoting best practice across the Trust and the wider local community.

To provide high level reports for the relevant hospital committees to determine trends analysis, benchmarking data and current strategies and initiatives towards improving identification and care of people with dementia, suspected dementia and delirium.

To lead and facilitate educational programmes to improve understanding of dementia and delirium through the development and implementation of specialist education programmes including identification, assessment and management in clinical practice for service users, students, clinicians and healthcare workers.
