Full Job Description

We are excited to be offering a 9 month secondment opportunity for a Band 5 facilitator to work in our Mental Health Access Service. Working in Portsmouth offers a diverse inner-city client base. We want you to come and join us helping our service users connect to the most suitable service for their mental health needs. We are expanding our team to provide a new service for Portsmouth parents working closely with family hub and start for life, which will enable quick access to a range of services for expert assessment and interventions. This includes a telephone line that residents can access, our team of call handlers will ensure we connect the service user to a suitable pathway linking in with a variety of other agencies.,

  • Provide advice, guidance, and support to the Health visitors and family hubs on mental health needs to support the management and treatment of their patients.
  • To work in creative and personalised ways to provide support, completing the work directly of value to the caller, ensuring their wishes and outcomes are always sought.
  • To work with indirect supervision and show extensive use of initiative in prioritising and making decisions in relation to effectively supporting the smooth operation of the service.
  • Work with the wider Access Team, wider mental health team and the Health visitors and family hubs to identify new opportunities to enhance and strengthen the relationships and develop pathways between both teams.
  • The opportunity for patients to follow a clear, helpful, and supportive process, where they would be screened and connected effectively to the most appropriate treatment or service related to mental health e.g., housing, financial and so on.
  • Undertake person centred interviews over the telephone or through digital platforms which identify areas where the person wishes to see change and/or recovery and make an accurate assessment of risk to self and others under the supervision of a registrant within the Access hub.
  • Ensure that the Portsmouth Mental Health Hub understand the provision available through the Health visitors and family hubs to best advise those contacting the service.

    £29,970 to £36,483 a year Please note, for part time hours the salary will be pro rata.
