Full Job Description

This is a new and exciting role within CHCP, to work in a rehabilitation unit setting providing high quality advice to patient's identified for rehabilitation., As an exercise practitioner you will work flexibly across the bedded units within the CHCP portfolio providing expertise in exercise prescription, integrating exercise prescription and progression in patient's rehabilitation programme whilst taking into consideration patient's comorbidities. You will consider the functionality of movement and meaningfulness of activities to patients holistic rehabilitation journey., You will be supported by well established MDT teams who provide rehabilitation within intermediate care, this includes Occupational therapists, Physiotherapists, and associate practitioners primarily.
