Full Job Description
An excellent opportunity has arisen for a Band 6 Falls Specialist Nurse within the Brighton and Hove Falls and Fracture Prevention Service.We are well established MDT, and we are proud in what we do - if you are an enthusiastic nurse and would like to specialise in falls prevention than we would like to hear from you.We are looking for a carrying and dynamic team player, with excellent written and verbal communication skills.The applicant will have good IT skills and be willing to travel as the job involves visiting patients in their homes and in the clinic setting.The post holder will be required to help in running Postural Hypotension Clinic under the Pharmacist guidance and to undertake multi-factorial falls risk assessments, both in the community and in a clinic environment., o To carry out the holistic nursing assessment/multifactorial falls risk assessment of older people who may be at risk of falls and fracture, and plan and provide programmes of care according to a person's needs and without direct supervision.o To manage a caseload of patients utilising specialist clinical assessment, diagnostic and treatment skills in the management of patients at risk of falls.o To support the delivery of the Falls and Fracture Prevention Service across Brighton and Hove in a variety of settings, including clinics and the patients' own homes.o To provide training and education to staff working across the local health and social care sector.o To coordinate and run the Joint Postural Hypotension Clinic in conjunction with the Community Pharmacist.o The post holder is expected to supervise less experienced staff and be able to teach qualified/unqualified staff and formal/informal carers.o To adhere to NMC standards and guidelines of practice, education, and conduct.
