Full Job Description

Do you have a passion for promoting healthy eating & promoting healthy lifestyles to children & their families? Are you able to motivate people? Do you have experience of working in health, local authority, or education? Would you like to be part of an exciting new integrated role working across both health & Public Health to support children, young people & their families to eat healthily & make sustainable changes towards healthy lifestyles?
We are looking someone who has experience of working with vulnerable children & their families, with an understanding of community-based health programmes working either as a family worker, nursery nurse or other health promotion roles. You need to be self-motivated, have experience of working in collaboration with partners & communities, & be able to lead groups to support & promote health & wellbeing. You must also have experience & understanding of data collection to be able to demonstrate the effectiveness of targeting interventions.
The successful candidate will be supporting CYP and families through their treatment journey (including parenting support), strengthening family relationships, working with services & partners to provide diverse support (e.g. access to financial advice), & development & improvement of the family support model. You will also be responsible for delivering group interventions for parents & young people to enable them to make sustainable lifestyle choices to reduce their weight & improve their health., NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex

High Cost Area Supplement - London
This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (minimum payment of £4,551 and a maximum payment of £5,735 per annum pro rata)., The Complications of Excessive Weight (CEW) is a public health lead initiative, developing a holistic approach to treating childhood obesity, including providing interventions such as family based therapy, and using culturally appropriate factors to personalise a support plan supporting dietary strategies for healthy eating and behaviours.
The post holder's responsibilities will be responsible for developing and delivering family support related to a two-year Complications of Excess Weight (CEW) clinic pilot. This will include- coordinating identified CYP and families, supporting CYP and families through their treatment journey (including parenting support), strengthening family relationships, working with services and partners to provide diverse support (e.g. access to financial advice), and development and improvement of the family support model. The post holder will be responsible for developing a directory of local services to be used as a resource by the team and the families. They will also be responsible for delivering group interventions for parents and young people.
The successful candidate will hold between 25- 30 children as an identified, selected cohort of children and young people and their families.
The post holder will achieve this by
1. Developing relationships with a range of partners working with the 0-19 population and their families in order to develop and deliver CYP and family- centred support.
2. Supporting the Operational Lead in collation of data and information in relating to CYP healthier weight and the B&D Children and Young People (CYP) Obesity Programme
3. Supporting parents/carers and CYP through the programme
4. Working with 0-19 Universal Services to provide a range of support and capacity building work in local organisations to increase support for participants on the CYP Obesity programme.
5. Scoping and coproduction of a tailored approach for CYP with -additional Special Educational Needs and their families / carers, 1. To deliver and provide leadership on family support within the B&D for CYP and their families / carers participating in the CEW pilot
2. To work within the Standard operational programme which includes maintaining Clinical governance aligned to all partners including BARTS health supporting the clinical audit in relation to activities.
3. To support members of the 0-19 team to deliver the CYP Obesity programme and create key links with other initiatives i.e. healthy eating.
4. Responsible for day-to-day project management re: delivery and meeting of agreed outcomes, in conjunction with the leads e.g. organisation, recruitment and delivery of required training and support
5. To escalate safeguarding concerns of concerns to line manager
6. To work autonomously both through working with CYP and families.
7. Development and piloting support for CYP with SEND, including leading the coproduction of a support package for CYP with SEND and their families in Year 1 and pilot it in Year 2
8. Clear communication supporting vulnerable children and families, including de-escalation
Leadership
1. To Provide leadership for the CEW working across NELFT, Community Solutions and other local partners.
2. Contribute and shape the development of awareness of CYP Obesity and specific programmes and interventions across Barking and Dagenham working in partnership with key professionals, team leaders and local authority partners.
3. To provide skills and knowledge to a range of professionals, community and voluntary sector groups and members of deprived communities and vulnerable people.
Clinical Skills and Job Expectations
1. To work with the 0-19 team to provide leadership for the CEW..
2. To support the 0-19 team in the training and delivery of health promotion in relation healthy eating.
3. Supporting the needs-led emotional, social and practical support to families of children diagnosed with health conditions In partnership with acute services
4. To support and contribute to the assessment of the needs of children, young people and their families in collaborating with families to develop goals and care plans.
5. Support parents/carers, siblings, and other family members with the emotional and practical adjustment to -healthier living Including healthy diet and physical exercise.
6. Support families in establishing effective routines around their child's needs and to integrate health care tasks into family life.
7. To promote families' strengths and motivations to support them in making positive behaviour changes and developing independence and autonomy.
8. To record and evaluate outcomes while continuing to develop the role.
9. To understand when to seek advice / assistance / report back to relevant staff.
10. To take responsibility for own workload and prioritise when necessary.
11. To attend and contribute to in-service training programmes.
12. To maintain appropriate digital clinical records in accordance with departmental policies., including use of emails and social media
13. To undertake duties as required to support the daily running of the service.
14. To provide information to patients and the general public as appropriate.
15. To be able to work in a variety of environments, across community setting inclusive of community clinics, Family Hubs, school and family homes .
16. To participate in regular supervision and appraisals.
Computer/Administration
1. Requires general computer skills, including use of Word, Excel and PowerPoint.
2. To maintain an up to date electronic diary that is available to colleagues Required to maintain personally generated data and information.
Communication
1. Responsible for personal communication across a range of agencies and organisations within LBBD and NELFT.
2. To contribute to the design and implementation of health education and social marketing campaigns for pertinent issues as and when required.
3. Required to contribute to training, seminars, and presentations across a range of agencies and organisations.
4. To develop rapport and establish collaborative working relationships with families using communication skills to resolve any conflicts or differences that may occur in that relationships.
5. To communicate effectively with all Multi Disciplinary Teams (MDT) staff across multiple agencies
6. To communicate in a timely and accurate way with colleagues about individual patients' treatment, progress or any change in the patient's condition by either by phone, face-to-face or e-mails with staff and colleagues from a wide variety of partner agencies.
7. To communicate sensitively with patients and relatives using verbal and non-verbal skills. This will include patients who may have difficulties understanding or communicating. For example, a person may have additional learning needs or be hearing/visually impaired.
8. To use skills of negotiation, persuasion and motivation to encourage patients and carers active participation in the management of the patient's care.
9. To maintain confidentiality in accordance with relevant policies operate within DATA protection .
10. To attend staff and team meetings.
Training
1. Responsible for supporting delivery and dissemination of multi-agency training in respect of the CEW pilot. This will include 1:1, workshops and nationally recognised programme material where necessary.
2. To take responsibility for own professional development as per NELFT/LBBD requirements.
Working conditions / Environment
+ The post requires activity to take place at numerous locations across Barking and Dagenham areas and therefore whilst an office base will be provided the post holder will need to be flexible with working arrangements The trust operates an agile approach to service delivery as well as a flexible working policy which must be negotiated with the line manager.
+ It is essential to have the ability to travel easily and efficiently across the patch.
+ The post holder will be required to transport training materials around the local area as required ensuring delivery of the planned interventions.
+ To be able to work independently with minimal supervision
+ Support will be provided through the line management structure of the 0-19 team on an ongoing basis.

This is an exciting opportunity for someone that wants to make a real difference to our local communities. Barking and Dagenham has some big public health challenges, including the worst childhood obesity in London and lower life expectancy than the England average. We are one of the 20% most deprived boroughs in England and child poverty is amongst the highest in London boroughs and the country (with 46% of children living in households on the poverty line). WE also have the highest proportion of children aged 0-17 in the UK. However, it is also a borough of great change - there is so much going on in Barking and Dagenham, even Hollywood wants to be part of it!
Barking and Dagenham's latest National Childhood measurement programme (NCMP) results 2022 indicate that 12.9 % of Children in Year R are overweight or obese, rising to 52.7 % of children at year 6. These are the highest rates of obesity in England, and result in our children experiencing numerous health and wellbeing complications. For this reason, Barking and Dagenham are piloting a specialist weight management service to tackle and halt rising obesity rates.
