Full Job Description

We are looking for someone who is interested in working closely with the Fast Track Clinical Team as their support officer. You will be someone who finds working with data exciting as a key to this role is the reporting and analysis of information for the team and senior management. You will not only have excellent interpersonal skills but will be a good communicator, keen team player and show initiative. You will be responsible for directly line managing a team of Fast Track Care Commissioning Support Officers whose role it is to source care packages for those that have been found to be eligible for Fast Track funding. We are a busy team who deliver excellent patient focused care and you will need to be able to flex and meet the demands on the team. You will track the patients journey through their journey while they are under the care of the team.,

  • Provide high quality project, service, initiative and administrative support to the Fast Track, standard NHS Continuing Healthcare and NHS-funded Nursing Care function within the NHS Continuing Healthcare team, including information and analysis.
  • The post holder will undertake reporting and analysis of information to support and improve Fast Track, standard NHS Continuing Healthcare and NHS-funded Nursing Care service delivery.
  • The post holder is a key member of the team whose overarching goal is to enable, promote and support the effective use of data, information, knowledge and technology to improve, inform and support the fast track team, standard NHS Continuing Healthcare and NHS-funded Nursing Care, projects, services, and associated initiatives.
  • The Post Holder will directly line manage the team of Fast Track Care Commissioning Support Officer's (CCSO)
  • The Post Holder will be responsible for ensuring daily trackers and systems are updated within a timely manner to enable regular and accurate data to be captured.
  • Provide the Team Lead and Senior Lead Nurse weekly updates of work through put of the team.
  • Tracking the progress of Fast Track and CHC Patients through the care process.
