Full Job Description

The post holder is responsible for the patient's journey from receipt of referral to appointment.The post holder will provide a proficient and effective system, in order to achieve the Trust's28-day faster diagnosis standard. To identify, analyse & resolve issuesrelating to the patient pathway and escalate any capacity issues to the Directorate. The post holder will incorporate the use of all the Trusts systems including E-Referral PatientAdministration system, I-portal & any other systems to achieve a smooth pathway for the patient alongside accurate and safe Data Quality collection.Most members of staff in the Trust have a responsibility for supporting the delivery of Planned Care Services, and the achievement of Cancer and RTT targets, whether in a supportive role, or in terms of providing hand-on care. Application of the national rules and guidance is critical in relation to treating patients in a timely manner and chronologically, whether by clinical priority or in terms of
length of wait. As part of the administrative support structure, the post-holder is responsible for adhering to the Trust's Access Policy and Standard Operating Procedures. Thepost holder will also require excellent communication and interpersonal skills and the ability to work flexibly is essential. The post-holder will ensure that confidentiality as outlined in the Data Protection Act 1998 is adhered to at all times., Undertake clerical duties, providing and receiving routine information requiring tact or persuasive skills. Contact patients via telephone to book their outpatient appointment in accordance to Trust targets.To set up ad hoc outpatient clinics as and when required.Communicate tactfully with patientsMonitoring the number referrals received, escalating any capacity issues to the Directorate to comply with waiting time targets.Ensure that the patient Access Policy is adhered to at all times and communicate any issues accordingly.Emailing information to patients regarding their appointmentTaking direction from the Directorate when booking patient appointments.Manually uploading the patient's document information from E referral onto Iportal in readiness for the patient's outpatient appointment. Deal with enquiries where there may be barriers to understanding.Required to maintain and keep current the skill sets required for the post Knowledge of Trust systems, E referral (RAS),
Careflow, Patient administration System Somerset and iPortal.Resolves issues relating to appointments, or any general enquiries.Record information provided by the patient when rearranging or cancelling anappointment or booking into a clinic.Plan tasks to meet service requirements.Ensuring that all new referrals received are logged ensuring a high level of accuracy on Careflow within the required timescales as per the Access Policy.
