Full Job Description

To work alongside and support other hospice services and external agencies. Receiving all external referrals and collating information. This includes contact with the referrer, requesting clinical reports, MDT information, consultations and consulting supporting professionals. To be the leading response to self-referrals/enquiries via telephone or other hospice services. Lead future FCS clinics within the hospice completing face-to-face assessment of needs following referral. Contacting service users, exploring needs, concerns, and expectations directly. Providing advice and support to the service user, ensuring they feel informed, listened to, and supported. Receiving all referral enquires, providing information and guidance upon accessing St Clare Hospice services or signposting to other agencies if appropriate. Supporting members of the volunteer sector when accessing services. To attend and when required Chair the daily triage outcome meeting contributing to the smooth transition of
referrals to services and a holistic view of needs. Maintain accurate nursing records via IT solutions in line with St Clare policies and within the guidelines laid down by the NMC. Participate within the allocation of IPU beds within the triage outcome meeting. To recognise urgent situations and escalate these appropriately to a member of the SPCT in a timely manner ensuring patient safety. To appreciate and respect a persons individuality, independence, and rights. Work proactively to maintain effective communication and demonstrating the values of the hospice. To be accountable for own professional actions in accordance with health and safety policies and adhere to fire regulations maintaining own personal safety accordingly. Attend meetings as required as a representative for First Contact Service. To be aware of own emotional needs, relating to the palliative care environment and how to meet them. Training and Education: To take responsibility for own ongoing education and
professional development. To support and participate in the education of colleagues within the referral process and promotion of services to both internal and external colleagues. Support and mentor new staff members and students. Develop training resources for mandatory and sessional training modules as required. Act as role model to other staff members enabling a shadowing process as relevant. Actively participate in quality improvement, clinical audit, and research as a means of monitoring and improving standards of care.
